Before we can begin providing services to your child, we need to discuss their needs and goals with you. To help us better understand your child's situation, we require the following information:

 

- A copy of the comprehensive diagnostic evaluation confirming the qualifying             diagnosis from a Developmental Pediatrician, Neurologist, or Licensed Clinical           Psychologist

- A recommendation or prescription for ABA with a diagnosis code signed by an MD

- A Letter of Medical Necessity, signed by an MD, with a diagnosis code

- If applicable, an Individualized Education Plan (IEP)

- A copy of reports/notes from prior treatments, including OT, PT, and SLP, if applicable

- A copy of the member's insurance ID card (front and back)

- A signed consent form for services and treatment.

 

Please note that any services related to Behavioral Health require prior authorization from your insurance carrier.
